In Case of Emergency

Emergencies will happen if you operate.  Being prepared for such events is reasonable and effective in mitigating confusion and converting a controlled situation to a chaos. You should have a simple step by step procedure that everyone in the room is familure with.  In addition regular drills to reinforce the procedures are helpful.  We will have these drills at the end of a routine and walk thru the steps to secure the bleeding, mobilize anesthesia and resucitation protocols, and shifting to the bedside to complete an open conversion.  

  1. Use anterior arm to hold point pressure with cigar

  2. Blood and cell saver in room

  3. Anesthesia staff in room

  4. Open thoracotomy tray/instruments/clamps/vascular suture (4-0 proline)

  5. Remove posterior and lateral instruments and de-dock ONLY those ports

  6. Move camera to assistant port and de-dock camera port. 

  7. Thoracotomy

  8. Remove anterior arm and replace with point pressure with sponge stick

  9. De-dock and move robot out of field

Anesthesia staff in room and massive transfusion protocols initiated

3.

Open thoracotomy tray clamps, 4-0 proline

4.

Remove POSTERIOR, LATERAL ports and arms

5.

Camera to assistant port, remove CAMERA port

6.

Thoracotomy

7.

Obtain open control and remove ANTERIOR intstrument and port.
